Referral
Referrals are made by calling the Clubhouse to schedule a tour and informational interview. A current Member will show you around and help you decide which work unit is a fit for your skills. Staff will also meet with you to learn your hopes and dreams both as part of the Clubhouse community and for your whole life.
Side by Side is an approved vendor for:
- Many Worker’s Compensation Insurers
- Georgia Department of Labor’s Vocational Rehabilitation Program
- Independent Care Waiver Program of Medicaid-Adult Day Services (PDF file)
If you want to refer a potential Member through one of these sources, please fax or mail the Side by Side Referral Form to 770-469-9385, Attention Member Intake and mark Confidential. If you choose to send this electronically, make sure it is sent securely over the internet.
